Practice level: EO

Scenario

You sent the team a written instruction explaining how to record a new type of enquiry. The following morning, Amira points out that you used an older version of the guidance. No cases have been closed yet, although six records were started using your instruction. Your manager asks in the team meeting why the work has paused. Amira is present, and you can correct the issue quickly by issuing the current instruction and checking the six records. You need to take responsibility, restore confidence and make sure the colleague who spotted the problem is treated fairly.
