Scenario
At a team meeting, your manager explains that filing errors increased last month and asks everyone to help identify the cause. Several colleagues become defensive and say the figures probably reflect mistakes made by newer staff. You know that two of the errors came from cases you handled. You have already worked out how the documents were placed in the wrong folders and can correct the affected records. The manager wants practical suggestions that will protect current work while the team reviews the wider pattern. You need to decide how openly to respond and what support to propose.
