Scenario
Two children in the same household have separate support cases. A medical letter for Child A has been scanned onto Child B's case because the family reference was used instead of the individual case number. The letter clearly names Child A. Guidance allows documents to be moved or copied after identifiers are checked, but the original location and correction must remain visible in the audit history.
Rate the actions

Check the child identifiers, place the letter on Child A's case, record where it was first attached and remove it from active consideration on Child B's case.
