Scenario
A medical report appears in an open case, but the name and reference number show that it belongs to another customer. A system-linking error placed it in the wrong record. Several colleagues can currently view the case. Information-management guidance requires access to be restricted, the incident to be reported and the document to be moved or removed through the approved process. The correct case remains open.
Rate the actions
For each action, choose Counterproductive, Ineffective, Fairly Effective or Effective.
Action A
Restrict access to the affected record, report the incident immediately, use the approved correction process and ensure the document reaches the correct case without creating extra copies.

Download the report, email it to the correct team and delete the downloaded copy after they confirm receipt.
