Scenario
Your team has found repeated duplicate uploads from a new scanner. Noor tells you privately that several occurred when she pressed the scan button twice because the screen appeared frozen. She asks you to remove the duplicates quietly because she is worried colleagues will blame her. Similar-looking documents can be separate valid evidence, so every deletion must be checked. You can reassure Noor, record examples, warn the team about a temporary precaution and report the fault to the system owner. You need to address the technical issue and help the team learn without turning the problem into personal blame.
