Scenario

A proposed system change will remove several manual checks and save staff time. It also creates a small risk that a failed data feed could leave some work unprocessed until an alert is investigated. Technical colleagues have produced a detailed assessment, but operational managers are interpreting it in very different ways. Some believe the change is unsafe, while others think the risk is too minor to discuss.
