Scenario

Your team regularly returns paper forms because customers leave the date field blank. Several colleagues say customers should read the form properly and see no reason to discuss it further. A new starter, Nia, suggests moving the date box and adding a clearer prompt. One colleague dismisses the idea because Nia has only been in the team for two weeks. You have records showing how often forms are returned and the delay this creates. The form owner must approve any change, while current cases still need a valid date. You want to improve the service and make sure useful ideas are considered fairly.

Rate the actions

Action A

Create a clearer standard explanation for returned forms and begin recording how often the problem occurs.
