Scenario

Errors have increased since a new workflow was introduced. Training completion is high, and the errors are spread across experienced and newer staff rather than concentrated in one person or team. Managers want an immediate refresher course, but audit notes suggest that mistakes may occur at different stages of the workflow. You have one week to recommend an intervention that will reduce the error rate.
